To register as a patient ask at reception for details. You will be asked to fill in a registration form. We will also invite all new patients for a consultation within 6 months of registering. 

Medical treatment is available from the date of registration. Please contact reception for further information.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. Please download a copy of our questionnaire: New Patient Questionnaire

You will be required to bring this in with some form of photo I.D. (e.g. a passport or driving license) and proof of address (e.g. Council tax bill, utility bill). If you are registering a child aged under 5, please also bring their vaccination record so that a copy may be taken to update our records.

Please note: it takes 7 days for all of your documentation to be processed and computerised, after which we are able to offer you an appointment.
